- This application must be submitted by the account holder or a person authorised to submit on their behalf.
- This form is for eligible pensioners who wish to apply for a rebate of up to 50% on the rent of their tenure.

- Prepare the supporting documents you may have to support your online application form.
Supporting documentation
- Evidence of authority to act where applicable
- Certified copy of both sides of the Pensioner Concession Card or DVA Gold Card
- Certified copy of drivers licence or identification card (e.g. NSW Photo Card) to verify your address
