- An Extension of the term of a Western Lands Lease in Perpetuity - Lease holder/s consent form (PDF, 180 KB) signed by all leaseholders – download, print and upload the completed scanned.
- Evidence that all mortgagees and or caveators are in support of this application
- Evidence of authority to act where applicable
